While we make every effort to ensure that our lists of doctors, hospitals, and other providers are up-to-date and accurate, our provider networks change on a daily basis. Accordingly, these provider listings may have changed. For the most up-to-date information, please verify whether your provider is a participating provider by either calling Member Services at the toll-free telephone number on your ID card or contacting the provider before scheduling your appointment or receiving services to confirm he or she is participating in CareSource’s network.
There are hospitals, doctors, and other providers who are not included in every plan network. Please make sure you are searching the provider network directory that applies to your health plan. There may be increased financial liability to you if you visit a provider who is not in your plan network or who was previously in your plan network, but is no longer in your plan network due to a network status change. However, your plan will not implement increased financial liability to you resulting from the removal or termination (network status change) of a provider or facility from your plan’s network or a change in a provider’s hospital affiliation until your plan’s provider directory has been updated to reflect such changes. If after your Plan’s provider directory has been updated and you are still receiving services from a provider no longer listed on the provider directory, then you may experience increased financial liability as a result of such provider being outside your plan network, except as applicable federal and state law and your plan’s evidence of coverage provide otherwise.
When you receive services at a hospital or other facility, there may be providers of services at the hospital or facility such as anesthesiologists, radiologists, and laboratories that are not in your plan’s network. Please call Member Services at the toll-free telephone number on your ID card for more information and complete details.
Services received out-of-network are generally not reimbursable. However, exceptions include: emergency and urgent care services; specific continuity of care concerns; referral by your primary care physician to a non-network provider because the specialty care you need is not available from a network provider; services you received from a non-network provider while in a hospital or other facility that is a network provider; or as otherwise required by applicable federal and state law along with your evidence of coverage. If you receive services out-of-network, then you may be responsible for all of the non-network provider’s charges and the non-network provider may bill you directly for payment of those services received out-of-network.
